Case Management

Parkview Occupational Health will work with your company to integrate case management services and offer a combined case management experience of more than 50 years. Your nurse case manager is an efficient communication tool to stay involved in the treatment of your employees.  Any case manager is available to discuss treatment plans. 

Nurse case managers evaluate each individual’s health plan and specific challenges while seeking favorable outcomes. The case manager’s goal is to: 

  •  Bring each case to closure quickly
  •  Increase communication between the employer and the clinic
  •  Decrease the fragmentation and duplication of care
  •  Enhance quality, and provide for cost-effective clinical outcomes

Nurse Case Managers use evidence-based guidelines, working closely with the primary treating physician in circumstances of short-term and long-term disability. Outcomes are evaluated to determine if gaps exist within evidence-based guidelines and actual care received.

It is the goal of your case manager to return your employee to work with as little amount of treatment days as necessary. All aspects of the case management guidelines point in this direction. The average number of days a person receives services at POHC, 15.8 days, is well below the national average, 34.3 days. Case management is the major contributing factor to these low numbers.
