To determine past and present exposures to hazardous materials and airborne communicable disease, some employees may be required to complete an OSHA questionnaire. This questionnaire is used to determine if further testing is needed such as physical exams, X-rays and pulmonary function testing. A provider will review every OSHA questionnaire that comes through our facility. This questionnaire will also determine the usage of a respirator mask.
